Contact details you provide and, once our portal opens, application information limited to what is needed to evaluate eligibility. We do not request Social Security numbers or detailed diagnoses.

How we use it

To respond to inquiries, evaluate applications, coordinate services with approved providers, and meet legal obligations. We do not sell personal information.

Sharing

Approved providers receive only the minimum information needed for an assigned referral. Applicant data is never used for advertising or analytics.

Application notice

We do not currently represent our program as HIPAA-covered. Retention periods and security practices will be detailed here before applications open.
